Fees
Intake Assessment (CPT Code: 90791) $250
Psychotherapy 55 Minutes (CPT Code: 90837) $200
Psychotherapy 45 Minutes (CPT Code: 90834) $155
Psychotherapy 30 Minutes (CPT Code: 90832) $125
Court Fees: $300/hour including travel time, time spent at the court hearing, and time testifying. Court fees are the full responsibility of the client as this is not a billable charge to insurance.
Payment is due at time of service and payment options include cash, check, HSA card, and credit card.
Insurance
I am in network with the following insurance companies: Avera, Dakotacare, Iowa Medicaid - Iowa Total Care MCO, Sanford (excluding Sanford Employee Plan), South Dakota Medicaid, TriWest (beginning 1/1/2025), United HealthCare, and Wellmark/BCBS.
Most insurance companies cover some of your mental health treatment. With your permission, I will help in the extent possible in filing claims and gathering information about your coverage, and you are responsible for knowing your coverage and for notifying Love of Play Therapy if/when your coverage changes.
Some helpful questions to ask your insurance provider before we begin could be; What is my deductible? Have I met my deductible? What is my co-payment? Does my insurance plan have a limit on how many sessions I can have per calendar year?
